This form is intended only for individuals to submit Data Subject Rights (DSR) request. By agreeing to submit a request via this form, you understand that Social Development Bank (SDB) will process your personal data to authenticate your identity and respond to your request.

Based on your request, SDB shall perform the following:

     - This form will be sent directly to the SDB Data Management Office

     - You will receive an automated email requiring your confirmation

     - Data Management Office will investigate and respond to your request in accordance with regulatory requirements and applicable Data Protection Regulation
